WHAT IS A DENTAL IMPLANT?

A dental implant is a biocompatible screw made of a special titanium alloy that can be used to make up for a missing tooth in a given area. During dental implantation / implantation, the titanium-based implant is screwed into the patient's jawbone, the gum is closed over it, and the implant is allowed to integrate into the bone tissue for 3-6 months.

Once the implant is integrated into the bone tissue, the gum above the implant is released and a gum former is inserted. After ten days, the gum former is removed and an impression is taken. For the implant head / abutment, the dental technique prepares the final prosthetic restoration, ie the fixed prosthesis.
 

The long - term fixation of the implant is not ensured by the screw thread of the implant, but by the  an extremely strong biochemical bond / bond between the implant and the bone. (Osintegration process)

After the ossification process, the final fixed prosthesis is prepared for the implant, so that the patient will be able to use the restoration for chewing indefinitely.

CONTRAINDICATIONS FOR IMPLANTATION

  • It cannot be implanted in acute illness, fever, severe coagulation disorders

  • Until the age of 18 because the growth of the jawbones has not yet been completed

  • If inflammation occurs at the intended site of the implant

  • During pregnancy

  • For 2 years after radiotherapy and chemotherapy

  • With minimal bone supply

  • In case of alcoholism, heavy smoking, drug addiction

  • In case of system diseases, immune disorders

  • When certain specific infections occur

  • In case of unfavorable anatomical conditions

  • For chronic oral mucosal diseases

SPECIAL SOLUTIONS

WHAT IS ALL ON FOUR IMPLANTATION?

In the case of few residual teeth in poor condition or complete toothlessness, the All On Four implantation technique can offer a stable-fixed denture solution where the denture is anchored on four implants in a tilted position.

During all on four implantation / implantation, we insert 4 16mm long titanium implants into a jawbone in a tilted position, close the gum above it, and wait 3-6 months for the implant to integrate into the bone tissue.

There are three types of prosthetic / prosthetic solutions available during recovery:

1. a temporary complete removable prosthesis is prepared for the patient, which can be worn for 12 days after surgery.

 

2. the patient already has a full removable prosthesis before the operation, which he can wear during the recovery period

3. If the primary stability of the implants is adequate after the surgery, a temporary but fixed prosthesis can be attached to the implants immediately after the surgery, even the old total removable prosthesis can be modified and fixed.

IMPLANTATION PROCEDURE, PROTOCOL

1. Meeting

Preliminary investigations and planning, CBCT referral handover.

2. Meeting

Our patients applying for implantation participate in a specialist consultation, which includes a medical history, a comprehensive clinical examination, CT scan and panoramic X-ray analysis, detailed information to the patient about the course of the implantation and the expected result.  

 

3. Meeting

The surgical day of implantation

- surgical insertion of the implant into the jawbone, healing screws are placed in the implant, the gum is sutured above it, so that when closed, the implant can be integrated into the jawbone without any problems. Time of osseointegration (takes 3-6 months).

- after the operation we check the position of the implants with a panoramic X-ray, then we perform a soft laser treatment, which  promotes wound healing

- we provide a special hygienic post-operative package for each patient.

4. Meeting in 7-14 days

The sutures are removed, the healing of the surgical site is monitored, and soft laser healing is performed to promote soft tissue healing, if necessary.

5. Meeting in 3-6 months

After the healing period, a small incision is made in the gums over the implant. The healing screws are removed from the implant and gingival screws are placed in the implant for 1-2 weeks. Gum-forming screws ensure that the implant is surrounded by an optimal amount of shaped keratinized tissue, which provides good cleanability and long-term stability. (keratinized tissue = gingival thick soft tissue around the implant)

6. Meeting in 1-2 weeks

The gingival screws are removed from the implant and  we take an imprint. After the impression is taken, the gum formers are put back into the implant.  Special impressions ensure that the technician can model the patient’s oral condition and make the desired prosthetic restoration under a microscope.

7. Meeting in 1 week

The final implant head set by the technique and the matte test of the finished crown and bridge are performed, in this state the crowns already have their external shape, but their glittering, enamel-like color world is still missing. During the matte test, check the contact between the crown and the implant head and adjust the height of the crown if necessary.

8. Meeting in 1 week

The technique performs the burning of the crown, bridge to light, in which case the crowns acquire their final shiny, aesthetic appearance. The final crown, bridge is tested and permanently glued.

3-6 months after gluing, the patient is called for a follow-up examination to check oral hygiene and the function and stability of the implants.

It is important to emphasize that successful surgery and recovery do not guarantee long-term success!

 

Without careful oral care for the patient, the implants can be lost, even the teeth!
